چکیده

We implanted a Medtronic Micra Transcatheter Pacing System (TPS) using standard technique, but after cutting the tether, a slight resistance was felt when the tether was pulled back. Subsequently, the electrical properties of the system were unacceptable. In order to retrieve the TPS, a 3 F snare with a 10–25 mm loop can be loaded into the delivery system. However, this snare was not available. Instead, the delivery system was retracted, and the introducer was left in the patient. A standard steerable sheath (Agilis, St Jude Medical) was introduced into the introducer, but there was a significant blood leak from the introducer’s haemostatic valve due to the incongruent sizes. Introducing a short 14 F sheath between the introducer and the steerable sheath solved this problem. A standard 6 F 20 mm snare kit (Amplatz Goose Neck) subsequently was used to grab the TPS (Figure), which could be released from the myocardium with a gentle pull and retrieved back into the introducer sheath. Echocardiography excluded pericardial effusion, and a new Micra TPS was implanted uneventfully. It is recommended to have a correctly dimensioned snare in stock during TPS implantation. In case, for some reason, TPS retrieval is not possible using a small snare, this report describes an alternative strategy.

Extraction of a Micra Transcatheter Pacing System: First-in-human experience

Clinical trials investigating the safety and effectiveness of transcatheter leadless pacemakers in humans are ongoing. These devices offer the benefits of cardiac pacing with the potential for a significant decrease in many of the risks associated with conventional pacing systems, including hematoma formation, pneumothorax, lead-related complications, and vascular obstruction. Human evaluation ...

External electrical cardioversion of persistent atrial fibrillation in a patient with a Micra™ Transcatheter Pacing System

We report a case of a 85-year old woman with a preexisting Transcatheter Pacing System (TPS) (Micra™ VR, Fa. Medtronic, Inc., Minneapolis, MN, USA) undergoing several external electrical cardioversions (CV) for symptomatic persistent atrial fibrillation (persAF). Due to bradycardia in the setting of atrial fibrillation a right apical TPS implantation was performed earlier. Four weeks prior to p...

Treating an infected transcatheter pacemaker system via percutaneous extraction

Introduction One of the most feared complications of a traditional pacemaker system is cardiac implantable electronic device infection. In such cases, a complete removal of the entire system is recommended. With the advent of the leadless transcatheter pacemaker system (TPS), the weak link— namely, the pacing leads that contribute to this problem— can be ultimately eliminated. At least 2 major ...

State of the art of leadless pacing.

Despite undisputable benefits, conventional pacemaker therapy is associated with specific complications related to the subcutaneous device and the transvenous leads. Recently, two miniaturized leadless pacemakers, Nanostim™ (St. Jude Medical) and Micra™ (Medtronic), which can be completely implanted inside the right ventricle using steerable delivery systems, entered clinical application. The W...
